| A 10% refundable deposit is required when booking. (Refundable if cancelled with 24 hours notice) |
| Packages must be agreed and paid for in advance to receive the discount. |
| Additional session packages can be purchased if booked within 30 days of the last session. |
| We accept local checks with Drivers License, cash, debit or credit card. |

| Important Notices: |
| 1. We require 24 hours cancellation notice or 50% of the session fee will be charged. |
| 2. An NSF Fee of $25 will be charged for returned checks. |
3. We are sorry but Brain Harmonizer training does not qualify under MediCare. Some insurance providers may include biofeedback training but please check with your plan provider prior to training.
